Thank you for your interest in becoming a Business Associate. Please follow the instructions below to activate your membership today:

1. Fill out and submit our Business Associate Membership Application form (PDF). Click here to view and/or print the form in advance and learn more about the membership options and associated fees. This form can be submitted to the Center online (via emai), fax or postal mail.
2. Pay for your membership and add-ons using our new Online Business Associate Membership Payment Form (below, on this page).
3. Once you've fill out out the payment form, click on the "Join Now!" button to go to our secure shopping cart, where you will:
   • Review the shopping cart page to make sure you selected the correct membership type. Complete the billing information requested in the form.
   • Use your credit card to pay for your membership. You will receive an email confirming your payment just a few minutes after submitting your information.
4. Your membership will be activated once both the payment AND the application form have been received.
